How Is Care Delivered
Membership Care
Membership is designed for ongoing, continuity-based care delivered directly in your home. Members receive scheduled wellness and follow-up visits, proactive health monitoring, and coordinated clinical support between appointments.
Each member is assigned a dedicated provider who becomes familiar with their medical history, medications, and long-term health goals — allowing for more informed, personalized care over time.
Membership also includes priority scheduling, reduced pricing on eligible add-on services, and structured follow-up to ensure concerns are addressed beyond a single visit.
Standalone Visits
Standalone visits are available for acute or one-time concerns and are subject to scheduling availability within the service area.
These visits focus on symptom-based evaluation and immediate care needs but do not include ongoing continuity support.
